Recognizing and managing an immune-related adverse event
- The gap
- Checkpoint inhibitor pneumonitis was being missed or mistaken for infection, delaying steroids and continuing immunotherapy inappropriately.
- What we built
- A multi-visit virtual patient case where the adverse event emerges across follow-up, with guided in-case feedback at the decision points.
- What changed
- Correct diagnosis more than doubled after feedback, and correct steroid dosing, holding immunotherapy and urgent referral all improved.

Choosing the therapy suited to autoimmune lung disease
- The gap
- Specialists recognized progressive fibrosis late in rheumatoid arthritis, and most left it untreated or chose a familiar antifibrotic over the therapy suited to the autoimmune disease.
- What we built
- A multi-visit virtual patient case with rheumatoid arthritis and interstitial lung disease, with guided feedback at each decision.
- What changed
- Choice of the suited therapy quadrupled after feedback, and fewer HCPs stopped the patient's rheumatoid arthritis therapy inappropriately.

Getting patients to a guideline LDL-C target
- The gap
- Treatment intensification was happening too slowly for patients who were not reaching target.
- What we built
- A longitudinal case tracking LDL-C across six months, where each learner's own decisions shaped the patient's trajectory.
- What changed
- Three quarters of participants reached the case target of 70mg/dL or below.
